Ballot By Mail Information
**APPLICATIONS FOR BALLOT BY MAIL MUST BE RENEWED EVERY YEAR STARTING JANUARY 1ST**
Ballot By Mail Eligibility Requirements:
- be 65 years or older;
- be sick or disabled;
- be out of the county on election day and during the period for early voting by personal appearance; or
- be expected to give birth within three weeks before or after Election Day; or
- be confined in jail, but otherwise eligible.
Instructions for submitting an Application for Ballot by Mail (“ABBM”):
- Fill out and print the ABBM form(PDF)
- Submit an order online and an ABBM will be mailed to you
- OR call our office at (936) 873-4425 and request an ABBM to be mailed to you
How to Fill Out the ABBM Form:
- Complete Sections 1 through 4.
- Sign and Date Section 5.
- If you were unable to sign the application and someone witnessed your signature, that person must complete Section 6.
- If someone helped you complete the application or mailed the application for you, that person must complete Section 6.
- Affix postage:
Address and mail the completed ABBM to the Early Voting Clerk:- If you printed the application, you must place it in your own envelope and add postage.
- If an ABBM was mailed to you - fold the application in half, moisten top tab, seal and add postage.

Changes to Ballot by Mail & Carrier Envelopes:
- Effective 2022- all Ballot By Mail applications MUST contain either your Texas Drivers License or Texas Identification Number, or the last 4 digits of your Social Security Number.
- All applications missing this information will be rejected.
- When voting by mail your ballot envelope or "carrier" envelope MUST contain the same identifiable information, if missing, your ballot by mail could be rejected.
